Please arrive 15 minutes early for your appointment for registration and to fill in a health questionnaire.

When you come in for your appointment, Remember to get the following:

• Insurance information
• Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits.
• If you have had surgery elsewhere, please bring a copy of your operation report.

Please call on (205) 930- 0080 during office hours and let us know at least 24 hours in advance so we can give that time to another patient.
